Pediatric Diabetes Program - Phase III

Purpose: Ensure continuity of care for children in the Pediatric Diabetes Program while accommodating the increased number of referrals into the program and conducting community-based research on effectiveness of interventions.

How:
  1. Create full time pediatric endocrinology clinic to manage care for children and youth.
  2. Provide evaluation and research support for obesity/diabetes interventions.
  3. Extend youth involvement with ENERGIZE and Drive 2 Fitness.
Outcomes:
  1. Continued measurement of enrolled children that include medical indicators and behavioral change.
  2. Sustained changes in medical indicators and behavior of youth enrolled in Drive 2 Fitness program.
  3. Increase in number of patients seen and managed through pediatric endocrinologist.
Cost/Term: $478,094
