Pediatric Diabetes Program - Phase III

Purpose:
Ensure continuity of care for children in the Pediatric Diabetes Program while accommodating the increased number of referrals into the program and conducting community-based research on effectiveness of interventions.
How:
- Create full time pediatric endocrinology clinic to manage care for children and youth.
- Provide evaluation and research support for obesity/diabetes interventions.
- Extend youth involvement with ENERGIZE and Drive 2 Fitness.
Outcomes:
- Continued measurement of enrolled children that include medical indicators and behavioral change.
- Sustained changes in medical indicators and behavior of youth enrolled in Drive 2 Fitness program.
- Increase in number of patients seen and managed through pediatric endocrinologist.
